Forms Overview

Forms help you collect intake information, consent, and feedback from clients. Create them from scratch or start from a template, then assign them to services so clients complete them before or after appointments.

The Forms Page

When you open Forms, you'll see every form with its name, description, and submission count. Search to find a form quickly, or browse templates for common business needs.

From the list you can edit a form, view submissions, duplicate it, assign it to services, or copy its link.

Creating and Using Forms

Click Create Form to build from scratch in the form designer, or Browse Templates to start from a pre-built layout and customize it.

Completed responses are stored in DaySpark. Open View Submissions on any form to see who submitted, when, and what they answered — or review submissions from the client profile or appointment drawer.
